Employee Benefits Summary 2003

UMCom cares for the overall health and wellness of its staff and is working hard each year to improve how it provides benefits. This year, UMCom (in cooperation with the General Agencies of the United Methodist Church) has provided a very generous benefit plan for each employee. Only staff working an annual average of at least 20 hours per week are authorized to participate in the full range of benefits. Below is a short summary of benefits that are offered:

Cigna Group Medical Insurance

  • EPP(HMO) Primary Care Physician must be selected - $20.00 co-payment for office visits - No out of network benefits
  • DPP(POS) Primary Care Physician must be selected - $25.00 co-payment for office visits. 90% of reasonable and customary charges paid until maximum out of pocket reached. Out of network benefits are paid at 70% after $400 deductible is met.
  • PPO In and out of network benefits provided. Office visit co-payments are $20.00. PPO network providers are paid at 80% after $250 deductible. Out of network charges are paid at 60% after $500.00 deductible. Out of pocket maximums apply to in and out-of-network

CIGNA Three-Tier Prescription Drug Program

  • Retail Service (30 day or less supply)
  • Mail Service (90 day supply)

Cigna Group Dental Insurance

  • Managed Dental Plan (DMO)
  • Traditional/PPO Dental Plan
